Why the lowest per-mg number is rarely the lowest cost
Per-mg pricing looks comparable and usually is not. Two pharmacies quoting the same preparation can differ on concentration, vial size and total content per fill - so the "cheaper" quote can cost more per treatment course once you map it to a real protocol.
